Thank you for your interest in participating in the Siena, Italy: Theatre and Dance Program (May 14- June 12, 2017). In order to be considered for participation, you must complete the following:

(Please note: if you are not a U.S. Citizen, it is your responsibility to obtain a visa to enter and leave the country you will be visiting. Not all programs require a visa, so check online or with an adviser if you are uncertain.)

Course Information

I understand that I must register for THR 5995 (3 credits). I understand that registration is billed through Academica at my appropriate rate. I must register for these courses within 30 days, prior to departure, or I am not allowed to participate.

Orientation

Payment, Deposit and Refund Policies

Program Fee: $3,608 which does not include tuition, airfare, personal expenses, passport and processing fees.

Program Deposit: The non-refundable program deposit of $408 is due by 2/2717. The remaining balance is due in two equal payments of $1,500 by the following dates: 03/17/2017 & 04/14/2017.

Withdrawal Policy: Withdrawals must be submitted in writing.

Refund Policy:

If participant withdraws 90 days or more before the start of the program, they will be refunded 75% of program fees less $608 non-refundable deposit.

If participant withdraws 60-89 days before the start of the program, they will be refunded 50% of program fees less $608 non-refundable deposit.

If participant withdraws 1-59 days before the start of the program, all payments are non-refundable.

Payment, Deposit, and Refund Policy Agreement
I have read and understand the Payment, Deposit and Refund Policies as listed above. I understand that I will be notified of my acceptance by an e-mail sent from the Office of Study Abroad and Global Programs that will be sent to the e-mail address I provided on my application. I also understand my acceptance e-mail will include a Program Acceptance Form that I must return within two weeks from the date of my acceptance letter or I will be withdrawn from the program, and agree to adhere to the parameters of the program as outlined by the Office of Study Abroad and Global Programs. I understand that in order to secure my place in the program, a $408 non-refundable deposit is due by 2/27/17. I understand that program fees may change due to unforeseen program increases. If this occurs, I understand that those fees will be passed on to me.

Release of Student Information

During the course of the student's participation in a study abroad program, the Office of Study Abroad and Global Programs may wish to provide relevant information from the student's educational records to the student's parents or other third parties. Depending on circumstances, information to be released might include student account information, information about the program in which the student is enrolled, or non-emergency information related to the student's health or safety.
