Student Center under $1,000 Food Safety Approval

This form is for food valued under $1,000. This form only needs to be filled out if the food requested is outside the guidelines listed on the Student Center Food Policy available at https://studentcenter.wayne.edu/reservations. Food Safety Approval Requests are not required for food/beverage for events totaling $1,000 or less as long as the event organizer is utilizing an approved local campus vendor.

Requests must be submitted at least 2 weeks prior to the date of the event. If the request is approved it will most likely need to have a second approval by the WSU campus food safety officer to approve the external food source.
