DLAR Campus Animal Transportation Form

This form must be completed in order to request Division of Laboratory Animal Resoures (DLAR) transport of animals between vivaria or lab spaces.

Transportation must be approved on your protocol. DLAR provides transportation on business days only. Submit this completed form at least 3 business days prior to the requested transport.

Cages must be labeled with purple transport acetate cards no later than 2:00 pm the day prior to transport. These can be obtained from the DLAR veterinary technicians.

DLAR cannot transport biologically hazardous animals, or animals with chemical hazards in the water bottle.

Chemical hazard transports will be reviewed on a case by case basis to ensure safety of personnel involved with transport.
