This form is used to transfer animals between protocols (i.e. to another PI, etc.). Cage cards must be updated within 48 hours of IACUC approval.

A separate DLAR Campus Animal Transportation form must be submitted if you wish the Division of Laboratory Animal Resources (DLAR) to transport animals between buildings.
