With your intent to bid, please include 3 local references (i.e. within 250 miles), a list of any other higher education customers you have within a 250 mile radius of Wayne State, financial reports, and any lost accounts.  These should be provided as separate documents and attached in the spaces below.

Firms that complete this form will be given password access to other documents, such as existing pouring rights and vending contracts.
