Please upload your excel class roster and your student's completed permissions slips as one PDF file. Failure to submit all completed permission slips by your specified deadline will result in the forfeiture of your registered spaces. 

• All permission slips must be legible
• Using cursive to complete permission slips is not recommended
• We highly recommend the permission slips being ordered to correspond with your excel roster

 Please complete this form once for every lecture subject area you are bringing students to attend. (Ex: If you have students attending U.S History, Chemistry and Physics--you would submit this form three times.)
